  4. Click on “Apply online for Disability Certificate” and fill out the application form with accurate information.
  5. Attach required documents, including: Recent passport-sized photograph, Identity proof (Aadhaar, voter ID), Address proof, Income proof (if applicable), Caste certificate (if applicable).
  6. Submit the completed application to the Chief Medical Officer (CMO) or designated Medical Authority.
  7. The CMO will verify your application and refer you to a specialist for disability assessment.
  8. The Medical Board will assess your disability and determine the percentage of disability.
  9. Upon approval, the Disability Certificate and UDID will be generated and sent to you.
  10. You can track the status of your application through the UDID portal.
