Upload required documents (scanned copies of medical certificates, Aadhaar card, and recent passport-sized photograph).
- Select the Disability Assessment Center. Choose the nearest government hospital or medical authority for disability assessment, if required. If you already have a valid disability certificate, upload it to speed up the process.
- Submit the Application. Review all entered details carefully before submitting the form. After submission, you will receive an application reference number for tracking your status.
- Medical Verification and Approval. If a medical assessment is required, visit the selected hospital for verification. The concerned authorities will review the application and approve the UDID card.
- Download the UDID CardOnce approved, you can download the digital UDID card from the Swavlamban portal. The physical card will be sent to your registered address by post.
